Please be aware that signing approval for an insurance company or other party to obtain information will allow them to get medical information about you from your medical record.
Chaperones
You are entitled to have a mutually acceptable third party present during internal or intimate examinations if you wish. You may bring a person with you, or we can provide someone such as a nurse, to act as a chaperone. Doctors also have the right to insist that a chaperone or third party be present, and may not conduct internal or intimate examinations if a patient refuses. In life-threatening situations these rights may be waived.
Transfer of notes
Under the Privacy Act, we must have a written request for transfer of medical notes to another practice. Please ask the receptionist at your new practice for a form.
